Direct-acting agents (Phenylephrine, Norepinephrine) are preferred. ✅ Echothiophate Iodide Eye Drops & Systemic Effects: Can cause cholinergic toxicity similar to organophosphate poisoning due to irreversible acetylcholinesterase inhibition, leading to bradycardia, bronchoconstriction, and muscle weakness. ✅ Severe Hypertension from Naloxone in Clonidine Users: Naloxone blocks α₂ receptors, leading to rebound sympathetic activation, causing a hypertensive crisis in patients on Clonidine. ✅ Bretylium Inhibits Norepinephrine Release: Initially increases norepinephrine release, causing transient hypertension, then blocks vesicular fusion, leading to hypotension. Used in refractory ventricular arrhythmias. ✅ Esmolol Removal from Blood: Rapidly metabolized by red blood cell esterases, not the lungs, giving it a short half-life (~9 minutes), making it ideal for short-term BP and arrhythmia control.
